We don't know her identity but we can assume that either his father and mother were Sarutobi's or just his father. Obito killed them when Naruto was being born I think

I think it was Asuma's brother or maybe a bastard child, but I like to think Senju bloodline somewhere along the lines was added to the Sarutobi.

We saw it happen with Uzumaki and it's normally in ancient tradition to marry people from different clans as a show of alliance.

I don't think it was a bastard personally, I think they would of made a bigger deal out of him being hiruzens grandson, or someone would of hinted at it. It's also completely possibly that it was asumas sister, and she wasn't really a nin, which would explain why she's never mentioned a bit at least
