The reason why he doesn't have his eyes is because those were apart of the Edo body and they disappeared with it.

Yeah Kishimoto didn't want to create some kind of loophole where suddenly there is a way to create TWO pairs of rinnegan in the world. Madara died without his rinnegan because he had given them away. He had the eyes as an Edo because they were his own and they're part of his body. Plus Kabuto figured out how to tweak them.

If the eyes had been destroyed maybe they would have come with his present body.

If he was given a body with a fresh pair, that would mean you could use edos to multiply coveted powers.

Madara was revived without his eyes because he died without them.

Madara was revived young either because Kabuto made his corpse young or because Obito used a Rinne Tensei on one person focusing all the energy, unlike what Nagato did who only revived people who died recently
