how did the first (shinobi no kami ) died ?

We don't know. Maybe his healing technique shortened his life span. I can't imagine anyone killing him, only Madara was capable of that and he failed.

lol... my best bet is that kakuzu and 1 more guy (that we don't know of) injured him so bad that maybe , that was the thing that made him get killed.

No he was not betrayed
bcoz he himself raid that he spoiled tsunade (his grand daughter)at early age. Also tsunade became student of third shortly after that.
Also gokage summit happened just after fight with madara.

He killed himself cause of gambling debts or Kakuzu wanted to have back his money and killed him, after he couldn't pay him.

I cant imagine someone short of the So6P or his brother killing him. Not with how his cells are so full of life force. I suppose everyone dies at some point. He wasnt that old though because he and Tobirama are around the same age.
