this is just a theory but im thinking that izanami and izanagi are counters for each other, if danzo is put in izanami he uses izanagi to get out of it, and vice versa. remember, if itachi uses izanami even unsuccessfully, he loses sight in that eye, and all the genjutsu that go with it.

itachi was very sick in his fight against sasuke against also has much less stamina than sasuke, who had his chakra restored by zetsu and was healed by karin midway thru the fight. it would be another very close match, but with KA up and ready and nobody to heal him, i think danzo would take this. itachi imo doesnt have it in him to go at it with danzo for ten-eleven minutes straight like sasuke did. he was also nearly blind during his fight against sasuke.

itachi is extremely powerful, but with his relatively low stamina, sick body, and half-blind eyes, he has everything going against him to fight a drawn-out slugfest against danzo shimura.

Mifune didn't break it, Danzo stopped controlling him after Ao found out that he's using KA.

Itachi can't break out of it considering he had his chakra disrupted twice and was still under the effect of KA. The first time was when Kabuto used the sound genjutsu, and the second time was when Sasuke put Itachi under his genjutsu.

The fact Danzo wanted to use it on Obito who he thought was Madara who had the EMS, and also Itachi planning on using it on EMS Sasuke implies that KA can't be broken by the sharingan/MS/EMS.
