Yes, he did. But if you looked again you'd notice the person beneath the ground never attempted to close the distance (perhaps scared he would be seen). Sharingan is not the Byakugan.

1.nope. Itachi attributed it to kakashi's speed of execution. If what you say is true they wouldn't even see past their own susanno.

2.he only saw the hole.

4.that's only available in legged susanno, based on feats.
