We all know Itachi was ill. We made tons of jokes about it being AIDS even. We don't know what kind of illness it was but it was a deadly disease. So Itachi was sick and so biologically speaking his eyes should've inherited that disease as well since they're a part of Itachi. So Itachi's eyes are diseased as well.
And my point is when Sasuke got those diseased eyes, again biologically speaking, that disease should pass Sasuke through those eyes unless Sasuke has some kind of immunity to that. Which is quite unlikely considering he and Itachi are siblings, so genetically they're not so different.

Will he die like Itachi too? Was that the reason he was away from his family at the epilogue ? He doesn't want them to see him like that dying perhaps ? What happens to dear Sarada ?

How do you even know his eyes are affected by the disease?

His eyes are genetically part of Itachi's body. He had a lethal disease, which carried to the last stage and killed him. He was coughing blood and all so it is obvious the disease spread. Most logical conclusion in this case that his eyes are carrying the disease as well.

You can't assume that without knowing what exactly is the disease.

This is internet forum. I am making logical conclusion with the information I have. It could be might as well that Itachi's eyes are healthy of course. But Itachi's eyes being diseased rather than healthy is more likely. Thats why I opened up this thread to see different ideas about it.

While under the tutelage of orochimaru sasuke would've grown immune to a number of diseases anyway. Biologically speaking you should really do more research on the human genome...before assuming that just because they were born similarly by nature doesn't mean they can't become differently by nurture. Plus there are many byproducts that could affect their survival rates anyway.

Om a side note, itachi was most likely mentally ill, seeing as how he has seen many deaths and even killing his own clansmen and mother and father with his hands he most likely became mentally unstable, visible in his fight against sasuke. Itachi most likely took drugs to suppress this mental illness (or possibly depression) but he may have overdosed causing other health problems. Meaning his somberness is an illusion as he may just be sick from the side effects of a possible overdose on mental pills, probably clozapine but that's just a guess.

And why is it ?

Well I explained it in the original post. Anyway Itachi's disease was at last stage which was the reason he died coughing up blood like that.

BZ remarked Itachi's movements being different than regular even, which indicates that his disease was at a level that he couldn't hide it.A seasoned shinobi like Itachi, he tolerated the pain of MS all those years should be able to hide the effects of disease just as easily and he did for a long time. But at his last fight he couldn't hide it so that means his disease was at the last stage.

When a disease is at the last stage it usually means that it spreads through the person's entire body. Eyes are part of that said body. There is a chance that disease didn't reached the eyes but the opposite is more likely considering the stage of the disease.

There are many fatal diseases. Many are dependent to different parts of the body. EX: Lung cancer is fatal but only in the lungs. Just cause you have a disease doesn't mean it affects the whole body. Without know the disease, anything we think is just a theory and only a theory. It can never become a conclusion.

Another note, diseases don't spread to the eyes. Eye diseases are only for the eyes and have nothing to do with diseases in other areas of the body. Only a few are known to affect the whole body and those are brain diseases as the brain controls every aspect of the body. Itachi showed no signs of any brain injuries or diseases and Kishi never hinted at any.

Taking everything the manga emphasized about it into account, it's very unlikely that Itachi's sickness/ailment was anything that would cause his eyes to be infected (and therefore be a liability to Sasuke).

Itachi could have had a heart condition, or something along the lines of an issue that is hereditary, but not contagious. If Itachi had something like a heart condition the only way Sasuke would get it is through genetics. This would also mean that others in Itachi and Sasuke's family would have had a similar condition ( like his parents, grandparents, etc)

Sasuke could already have the condition, if it is hereditary, he just doesn't have symptoms. However, he might also not have the condition. It's a flip of the coin really when it come to genetics. So if Itachi's disease isn't contagious Sasuke and anyone who comes into contact with Itachi's body fluids would not contract the disease.

If the disease is contagious then Sasuke would most likely have the disease, especially since he came in contact with Itachi's blood.
