Sakura is easily stronger then the weakened Tsunade who couldn't use her full power because she was afraid of blood, therefore I'd say Sakura wins this with mid diff.

I agree with most of this post.

Tsunade didn't even use auto healing against Kabuto. She activated creation rebirth once Tsunade got stabbed by Orochimaru when she went in to protect Naruto.

I say Sakura wins mid difficulty with Katsuyu and high difficulty without. Although Kabuto has very good chances at winning if Sakura doesn't summon.
