What if the Ramen Guy was always the Sage of 6 Paths in hiding, watching over Naruto, who he knew would be the child of prophecy. We haven't seen his Rinnegan because of his closed eyes. Then, one day, once the Juubi begins annihilation and stops derping around, Ramen Guy comes back and owns it.

Honestly, Sasuke becoming hokage instead of Naruto, that's a twist for you.

If you want a serious answer I'd say that the eye would get rejected since it's a human eye. and also it wouldn't fit in the first place. Humans can receive pig skin amongst other things via transplantation but the eye isn't similar enough.

I'd say if it were not for him being crazy Sasuke would make a far better hokage than Naruto.
