i'll tell you how you get the 3rd eye , which is: having both rinnegans , being the juubi jinchuriki and having the tree sucked up in to you and that's how you get the 3rd eye U_U.

Kaguya had different eyes than Madara, Hagoromo and Indra. She possessed the nine tomoe and the byakugan and may have mixed their powers to create different techniques. It also looks like there were two moons during the time of the Ootsutsukis.

........I do not know, but much of the events could have been misinterpreted. Like the sealing stone could have been a smaller body/planet that was sent to the existing moon and sealed there. That is one interpretation from my side.
