Because the poison would still paralyze his brain. When his head was cut off he wasn't able to move his body.

She said herself that tsunade was the one figuring out the antidotes to her poisons in the 3rd ninja war.

I mean. His body should have had multiple system failures once the head is removed yet his head was only stiched on (not even connected properly) and he started moving again. So my guess is that the only system that needs to work is the nerve system.

Whether her poison has any affect on his body let alone his nervous system we wouldn't know. What we do know is hidan loves cqc something that puppeteers aren't good at. Once he gets her blood..gg
