At first I thought perhaps it wasn't Kakashi and I still don't believe it is entirely his fault, but I was thinking, what if Ao saved Rin and Kakashi from Yagura and other Mist ninja, losing his eye in the process, but ultimately living and allowing Rin to transplant an eye from a fallen Hyuuga. Then I'm thinking Shisui was the team-leader or something and had Rin killed for betraying the village and giving away the Byakugan to an outsider by temporarily controlling Kakashi with KA and then himself attacking Ao. I think Ao then managed to flee and wound up leaving Yagura's side of Kiri and joined up with Mei and the rebellion ninja, which eventually won the civil war.
Now, some may say this is farfetched and I am drawing at strings, but I'm just trying to fill in some blanks. For one, Rin is clearly being killed by a Chidori to the chest yet there is no Edo version of her, meaning either nobody wanted her back (which is not true) or possibly Shisui disposed of the body like he did his own, leaving no trace. Could possibly be Shisui's second Mangekyou ability.
Shisui using KA to control Kakashi would fill in a few more spots. Obito would obviously want that eye because he saw what it could make Kakashi do and how else would he know what KA is capable of without seeing it in play? Shisui is said to have fought Ao and Ao knew just what Shisui could do with his eyes. Rin also transplanted Kakashi's eyes and where else would Ao get a Byakugan eye from?
Summary: All that I've said is just me rambling, but what it finally comes down to is: I think Shisui was there (as well as Ao - Mist ninja with the Byakugan) and played some role in Rin's death whether that be the cause of it, or at the very least the disposal of her body.
