Noctis walks in and reads the wishes on the patient that were put on record when he was taken in. "...Ummm, uhu...... uuuh, uhu..... ah, ok. Alright, I will have to get something.."
He disappears for a while and then suddenly returns out of nowhere with chains that normally would be attached to chainsaws. "Right, mister...... Castiel. This is going to take a while. Please make sure you sit still so I will not cut your leg off by accident~"
Noctis starts focusing his chakra into one of his hands. He focuses it so that the chakra is aggressive, with the intention to kill. Once he gathered enough, he starts to release his chakra slowly and steadily so that it will not hurt him, but only kill the micro organisms on his hand. He then strikes his hand over his other to kill all the micro organisms that are potentially dangerous for his patient. When he is done with his first hand, he immediately repeats the procedure for the other hand as well.

Description: The medical shinobi fills his palms with small amounts of extremely aggressive chakra and runs them over the areas of his body he wants to clean of any microorganisms, harmful substances, etc. The chakra will eliminate any inert harmful substance or microorganism present on that area. Normally this technique is done on the hands and lower arms just before having contact with a patient but can be applied to the whole body in specific situations (before performing surgery, if the enemy is highly contagious, etc). After being done, the chakra automatically turns back to his regular, non-harmful and calm one.
Now his hands are clean, Noctis can almost start the procedure, however he has to make sure both the patient and the chains are clean before he can combine them, otherwise the result could be disastrous for the patient. He once again gathers the same aggressive chakra into both of his hands as once this time, like it is nothing. He slowly and steadily starts releasing it and puts his hands on the legs of his patient. By doing so, he cleans the patient's legs from potentially dangerous micro organisms and making them sterile. Once he is done with the patient's legs, he does the same to the chains. Even though he already washed them thoroughly, you can never work clean enough.

Description: The medical shinobi fills his palms with moderate amounts of semi-aggressive chakra and runs them over any areas of the patient’s body he wants to clean of any microorganisms, controlling the chakra flow constantly so it doesn't becomes too lethal. The area affected is 4 times the area of the users hands. After being done, his chakra automatically turns back to his regular, non-harmful and calm one.
Now everything is clean, Noctis is ready to begin the procedure, however since the surgery would cause too much pain to the patient to handle and lie still, he decides to first create anaesthetics to lighten the pain for the patient.
To make the potion, Noctis first needs to summon some equipment. He gets out a scroll and bites his thumb after which he wipes some blood on the scroll and heals his little wound. As the blood is wiped, a mortar, pestle and a spoon appear out of a puff of smoke.
Noctis puts a very tiny amount of Valerian in it and mixes it with water. As soon as he is done, he puts some of the potion on the spoon and gives it to his patient.

Description: The medical shinobi bites his thumb and draws his Medical Kanji on the Medical scroll underneath the symbol of the tool he desires to summon.

Noctis waits a short while until the Valerian kicks in, sighs deeply and starts to gather more of his chakra, however, this time the chakra is not as aggressive. It is meant to leave everything as it is, but just separate some tissue without affecting it too much. The chakra grows stronger as he keeps gathering it, until he has enough. Then he puts his hands on the right leg of the patient, channelling all his gathered chakra into the bodily tissue located on the leg. At first it seems like nothing is happening, but then the flesh suddenly starts moving gently. As it separates it gives Noctis the opportunity to perform surgery on the leg without causing too much damage.

Description: The user places a palm on the patients body, pouring their chakra into the tissue, gently moving it apart to create an opening to allow him to perform surgery on the patients internal organs without creating an incision. In fact, the technique separates tissue in a non traumatic way, avoiding damage and relinquishing the need to heal the tissue afterwards. The opening closes virtually instantly after the chakra input is stopped. However, even if it doesn't do damage, the usage of the technique causes tremendous pain to the patient so the use of anesthetics before the procedure is highly recommendable.
Note: User must have their hand on the patient for a full turn before this takes affect and must maintain constant physical contact and chakra supply to maintain it. However, the technique can be done in tandem with others such as chakra scapel or mystical palm.
Now with the leg's tissue separated, Noctis is able to put the chain in easily. He places it carefully how the patient said he wanted it and then releases the chakra used for the Soshiki Bunri so that the wound rather quickly.
Looking quite proud of the result, Noctis immediately starts to gather more of the same chakra, and focuses it in both of his hands. Now the first leg seemed to be successful it is time for the other leg. Having gathered his chakra enough, he starts to release it slowly and steadily whilst putting his hands on the patient's leg. After waiting some time, the tissue of the second leg also starts to separate, granting the same opportunity as before.

He grabs the second chain and carefully places it between the flesh of the patient, making sure it is exactly placed as in the first leg so there will not be any complications in later use. When he has placed it correctly, he stops the chakra flow into the leg which causes the wound to close as if it had never been separated.
Noctis takes a step backwards once the wound is closed again to see if he did well. As it appears there are no complications and the work of the placing is done, Noctis starts to gather more chakra for the next part of the surgery.
The placement of the chains is not the only thing that needs to happen, the patient wants to be able to control the chains, therefore the nerves and the chakraflow need to be connected to the chains. The chains need to become a part of the user's body. To ensure that the patient will be able to use them, Noctis once again places his hands on the patient's leg and releases his focused chakra into their flesh.
(Iryō Ninjutsu: Saibō no saisei) Medical Technique: Cellular regeneration

The drawback of this technique is that its not actually a healing technique but a tissue growth technique. It can serve to grow missing tissue but it cannot heal in the truth essence of the word and in essence is a much simpler application of the Mystic Palm technique.
Noctis then uses the chakra that he releases into the patient's body to take control of their cell cycle. He then makes the cell cycle go faster and so regenerates or grows the nerves and other vital parts in the patient's legs to fit the chains. Once done with the first leg, he immediately starts with the second leg, going through the same process again.

To help the body heal the wounds created by the surgery and get adapt to the chains, Noctis decides to heal the legs some more. He first gathers more chakra in both of his hands until they start to glow aqua-like blue. Once he gathered enough chakra, he lays his hands down on the patient's legs and channels the chakra into them. The chakra will go through the area where he performed surgery on and will heal the parts that are still injured or missed by Noctis.

Description:
This medical ninjutsu allows the user to speed up the body's natural healing process by focusing mainly Yang chakra onto their hands, forming an aura of greenish healing chakra which he then sends into a wound or afflicted body part. This allows the user to heal a patient without the need for medical equipment or surgery, making it very useful on the battlefield. It can be used to treat both external and internal injuries. It is vital to match the amount of chakra used to the severity of the affliction or injury. This requires a great amount of chakra control. Because of this, only a few highly skilled medical-nin are able to use this technique. The limitation of the technique is that it cannot regrow missing body parts or deal with poisons or toxins, though it can heal its damage. Additionally, perhaps its more severe drawback is the chakra cost, which, as the technique stands as a jack of all trades in terms of medical ninjutsu, is much higher than any other medical technique. For each damage point the user of the technique wishes to heal he needs to spend twice as much chakra. Basically, for X amount of damage the user needs to spend 2X amount of chakra to repair/heal. The user can also use this technique on himself and it can be used with one hand or two and even in two targets at a time or while using another technique with the other hand. The speed of the healing is fast but not instant, specially in internal injuries.
Noctis takes a step backwards and looks at his patient. "There, I think we're done. Now, stay in bed until the Valerian stops working, which I think will be a couple of hours, then rest for one more day to make sure everything is healed. Be sure to not overdo it in training for the first weeks! I'll be going now, have fun with your chains!"
