Tsunade had to have been 10x stronger in her prime. As all her feats shown now have been with a 70 year old body that only looks younger.

Most probably in relation to his chakra scalpel. His accuracy surprised her as he had cut off the muscles around the respiratory area. It's kinda weird though, as we have never seen her use that technique.

Up until that point, the only ninjutsu Kabuto had shown Tsunade were his chakra scalpels. It follows that Tsunade would have been referring specifically to his use of chakra scalpels alone.
