Its not. One eye has Enton, the other amaterasu. It was confirmed in the 5 kage summit arc. It was reconfirmed when itachi used tsukyomi to break sasuke out of kabuto genjutsu and sasuke used regular genjutsu

The whole point of Tsukuyomi is to mentally incapacitate you through extreme pain.

You cant tank a genjutsu, since its not reality. And I thought once you are in it you cant get out unless you have MS or someone disturbs the chakra flow

No. I'm saying that Itachi has limited control over his Amaterasu, just not to the extent of defining it as "Enton" just as Sasuke has a watered-down version of Tsukuyomi simply called "Sharingan Genjutsu" because it is "a far cry from Itachi's Tsukuyomi."
