The problem here is the limited mobility handicap on Hidans ritual. He has to stay in that small circle for it to work. With his mobility hindered Sakura could close in and attack without him dodging.

But at the same time she would just end up punching herself as well due to the ritual. Decapitation might work, but that leaves Hidan pretty much helpless aftwards....

Damn it's almost like a stalemate. I guess it just depends who has the most chakra and endurance at that point.

You don't die the moment your heart is pierced, so there's room for Byakugo to activate and heal them.

Hidan's curse > Whatever the **** sakura/tsunade uses.

When he pierces his own heart, he usually keeps the stake in there which means there is no room for Tsunade/Sakura to heal because of an object obstruction. Simple as that.

Byakugō can heal any organ and most injuries, not to mention it’s activation is instamt. So Sakura and Tsunade destroy.
