if "the moment madara was revived" means when he got rinne tenseid by obito, then Naruto and edo Minato are all that's needed.

If it's when he was revived as an edo tensei, then Minato, Bee, and Nagato would probably be enough to give him a hell of a fight. Add Tobirama and someone else with super firepower like oonoki or muu and it's a slaughter.

Chouji in Butterfly Mode can wrestle with his PS and Restrain it, with Karin supporting him via Chains.
Sasuke can Summon Aoda, The Three Serpents, and Manda to assist with that, which, considering you didn't give Madara Kurama, should be enough to make his Susanoo too slow to properly function.
While he wrestles to break loose, Sakura can batter the heads ferociously, with Mei melting the rest of the Susanoo. Eventually, it will have Zero Choice but to break. He'll have to engage in a CQC which will admittedly, immediately dispatch Mei and Chouji.

But Sadly without Sensing, it only takes a minor distraction, I.E. Sakura pouncing out of nowhere, to have him slip up and get stabbed by Sasuke.

To tie up loose ends,
-each combatant will have two Katsuya mounted Ma and Pa Style, relaying info and healing.
-Sakura will enhance Chouji with Byakugou
-Katsuya will assist Mei with Acid.

I sincerely didn't understand what Karin is here for. Anyway Chouji cannot wrestle with PS, cause it's even stronger than Bijuus, and the fact Mei was melting Sasuke's Susanoo doesn't grant she can melt Madara's Perfect Susanoo, I personally don't believe so. Sakura gets molested in zero time, Sasuke's the only that can oppose some resistance but alone he can't beat Madara at all.
You're referring to Katsuyu, I guess, because she informed Naruto step-by-step during his clash with Pain, but she being that informed about Six Paths doesn't imply she knows the same about Madara. And anyway, Sasuke is not as good as Naruto in strategy

Karin is there considering her War Arc Self can break the entire Mini Shinsusenju, Can heal someone fully at least twice, and is the best sensor in the manga. That is where the intel is coming from, and she can relay that information seamlessly using Katsuya (Who also makes Genjutsu null and void).
A normal BM Chouji was able to wrestle the Gedo Mazou. Even though I'm sure PS is stronger than that, a Byakugou enhanced BM Chouji, Made possible by Sakura's being on the team, will he able to go toe to toe with PS, that will be being slowed down by Karin's Chakra Chains, and Restricted by boss Summon Snakes.

Sakura, with Byakugou activated, should be able to survive PS Shock waves, which probably won't be needed seeing as the Susanoo will be locked in place by all the stuff holding it down. Remember that Gamabunta was as big as Kurama when not putting the Tails in the equation. Aoda, the Three Serpents, and Katsuya are all as big as Bunta. PS is about as big as Kurama. He isn't going to break free.
This would leave him to either sit while Mei, Sasuke, and Sakura pound at his Susanoo, which would mean instant death for Madara as soon as it breaks, or for him to release his Susanoo, and fight head on.
In a head on Fight, if Karin is sensing, and he is being continuously healed by Katsuya, Sasuke will beat Madara.

Madara would beat your team evem easier than Gokage. Only one worth of fighting is EMS Sasuke
