Sasuke for not following through his plans

Fugaku for planning a coup instead of just negotiating

Tobirama for making edo tensei.

Itachi for thinking Sasuke would return to Konoha like normal after he filled him with hate.
Hashirama for not killing Madara. (I think that it will be revealed that Hashirama and Madara were friends.. just like Naruto and Sasuke)

I don't know if Kakashi actually "let" Rin die. We don't know how she did. Obito is just claiming that he did, but he may have just felt responsible for her death for not being responsible enough to save her.
