Basically this. I doubt that the Naruto and Karin are very closely related. Same goes for the majority of the Hyuuga (excluding Hinata and Neji, they are cousins)

Don't forget these clans are constantly expanding and diversifying their bloodlines. For example, is every Uchiha Clan member really an Uchiha.. if so the entire family has been commiting Incest for generations so in this verse it doesn't matter.

However, there has been a millennium's worth of time for the genetic lines to diversify. The inherent risks involved with deformity don't really exist here. The blood lines have diversified so many times over the years for all we may know they could even be from parts of the blood lines that are actually just associated through side marriages and such.
