The perfect Dojutsu is the 9 Tomoe Rinnegan sported by Kaguya. To get there one starts with basic Sharingan and faces pain and sacrifice until the Perfect 9 Tomoe Rinnegan is reached.
The pain, loss, or sacrifice required becomes progressively worse with each step
1) Suffering causes the Uchiha to gain Tomoe and awaken the basic Sharingan.
2) Loss of a loved on causes the Mangenkyo Sharingan
3) Loss of a sibling and a transplant causes the EMS
4) Suffering near death along with combining Ashura and Indra chakra allows eyes that have once obtained the EMS to transmute into the Regular Rinnegan
5) By dying and using a surrogate to hold the Regular Rinnegan( i.e Nagato or Obito) and making them use Rinne Tensei, one can come back and re-obtain the Regular Rinnegan and use it's full powers.
6) By sacrificing humanity itself and forgoing all connections with humans, and by getting close to the moon, one can obtain the 9 Tomoe Rinnegan.
7) Finally, by facing betrayal from the "self"( fake self that is BZ) and completely sacrificing ones life and all identity, one can obtain the Perfect Dojutsu: Kaguya's 9 Tomoe Rinnegan by transforming into Kaguya herself. This step is completely involuntary, and hence is the ultimate sacrifice.
That Final Rinnegan used by Kaguya is incredibly powerful, more so than the one Madara obtained. Madara's Third Rinnegan needed two regular Rinnegan and the Moon to support it's existence. Kaguya's Rinnegan doesn't need the moon nor two other Rinnegan as support. This allows her to use the Byakugan since she doesn't need to have two extra Rinnegan.
Notice that there are 6 steps followed by a 7th step. The "outer" step. "Outer" meaning literally obtained via and outside entity by becoming the outside entity.
Again the only way to reach the final pefect Kaguya Eye is to finally be betrayed and discarded. The ultimate loss
