It was a mistake make by the people who cleaned up his panels. They thought it was supposed to resemble the Sharingan so they made it like that. I can find the original panel if need be-

Also I'm guessing it was the original design for the rinnegan as it is the next step after the sharingan. But I think kishi scrapped it because it was to obvious.

I have a theory how the sage obtained the rinnegan from the juubi and removed the hated (reddness and tomoes) and became a burden of that hatred (neckless of red tomoes around neck).
