I've decided to post this thread because of the wide array of opinions on the subject of how Amaterasu works. I hope to demystify Amaterasu using clear evidence from the manga and by doing so, dispel these two common misconceptions:
1. That Amaterasu is fired out of the eye as a projectile.
2. That Amaterasu manifests at the focal point but never acts as a projectile.
Upon examination of the evidence you should come to the conclusion that Amaterasu
1. Does indeed spawn at the location of the user's eye focus.
2. Also acts as a projectile if Amaterasu doesn't attach to an object/burnable medium - moving in the direction of the what the user's eye gaze was when it was spawned i.e. in a straight line away from the caster.
First of all, we have Itachi's use of Amaterasu:
Secondly, Sasuke's use of Amaterasu:
There are many other examples of Amaterasu being used in the manga. Keep in mind, you never see Amaterasu coming out the eye. What you do see is the eye activating, then the appearance of black flame some distance away (whether it spawns on the intended target or spawns short of it).
To finish off, here's an example of amaterasu instantly spawning at the target in both the manga and anime - (

First of all, we have Itachi's use of Amaterasu:
In the top left panel we have an example of Amaterasu acting as a projectile, flying down into the trees.
We also have two examples in the middle left and bottom right panels of Amaterasu manifesting at Itachi's point of focus. As the point of focus moves, a trail of expanding flame is left behind in its wake.
The trail of expanding flame proceeded to act as a projectile, and fly into the trees below:
As we know, use of the mangekyou weakens the eye and affects its ability to focus correctly. This explains Itachi's extreme difficulty in hitting Sasuke directly with his focal point.

Co-ordination of the eye muscles while using amaterasu was extremely demanding on Itachi.

vs Raikage
vs Bee
At the Kage summit Sasuke fired an Amaterasu, and Raikage managed to flicker to the side before it stuck to him. It then flew into a samurai who was standing behind Raikage's previous position.
These two pages are most commonly used as an argument that Amaterasu is a projectile. And I totally agree; it is a projectile that can be dodged if it hasn't attached to anything.
You may ask yourself, "how can you dodge something that manifests itself on you?"
The problem is Sasuke had similar eyesight problems to Itachi, related to use of the mangekyou. The focal point wasn't touching Raikage. It was actually a short distance in front of him. If you are doubtful of Sasuke having eyesight problems, I direct you to this:
Sasuke perceived the glass as being closer than it really was. His depth perception was off.
Back to the fight:
A small ball of black flame appeared a short distance in front of Raikage's chest. Raikage only reacted to it when it was inches away from him. There are no signs of it actually sticking to him.
If Amaterasu came out of Sasuke's eye, why would Raikage wait until it almost touched him to react to it?

Here we see Bee raising his tentacle towards his face in the last panel in anticipation. There is no sign of amaterasu coming out of the eye. Bee probably sensed the pressure that Nagato spoke of, and raised his tentacle defensively. The gradual zoom in indicates Sasuke's efforts to focus in on Bee. He was trying to get a direct hit on Bee's face. During this time interval Bee raised his tentacle in front of Sasuke's line of sight. Then, Sasuke spawned the black flames.
We see that the fire appeared without any kind of trail leading to sasuke. It clearly expanded from a central point on Bee's tentacle. This is the point where Amaterasu spawned - its point of origin.
