Am I right in thinking EMS Sasuke would beat Itachi 9 times out of 10?

Yes a sick Itachi<EMS sasuke, Itachi was sick was constantly using chakra to will himself to stay alive because he was constantly focusing on it that uses chakra thus all his energy went to that since almost everything the narutoverse uses chakra in some form or another. so ya in your stacked encounter sasuke wins with high difficulty.

I think it depends if the fight takes long or not. Itachi can try to outsmart him and win, but if the fight takes long, Sasuke will win cuz of Itachi's low stamina.
You can even notice that during the fight against Kabuto, Sasuke was even worried about Itachi. He asked him if he can still continue to use Susanoo:

That means he knows Itachi has a low stamina normally and he could take advantage of this if they eventually fight by spamming

Yeah I think EMS Sasuke can beat sick Itachi with mid difficulty. EMS Sasuke can also beat healthy Itachi with high difficulty. Edo Itachi, however, would beat Sasuke high difficulty even if Sasuke could seal him.

I say this because genjutsu is not going to do anything in this battle. Even though Itachi is smarter than Sasuke, this battle would be similar to Kakashi VS Sasuke.

Now Sasuke has more flashy moves than Itachi. Not only can Itachi not put up susanoo for a longer period of time than Sasuke, he also does not have any techs that can get through Sasuke's susanoo. Sasuke, on the other hand, has kirin and enton which are more dangerous than any of Itachi's techs. Both bros will be forced to use amaterasu against each other which will be in Sasuke's favour since he can use kirin every other minute.

Kirin doesn't even require much chakra once it has been prepped so Itachi would be forced to drain lots of chakra only for it to benefit Sasuke.

I know for a fact that Itachi is superior to Sasuke in most categories but let's face it, it will come down to who is more powerful in the end and that is Sasuke.

Itachi is most effective when the enemy is unsuspecting but in this case, Sasuke knows everything about Itachi. I don't see how Sasuke will lose to Itachi unless Sasuke becomes too cocky and tries something very stupid.

I've noticed that now that Sasuke has EMS, his techniques drain less chakra and he doesn't suffer as much pain as before.

Saying all of this I would still put Itachi above Sasuke on a top 10 list because I believe Itachi has a better chance at beating other characters like Minato and Nagato then Sasuke.

Itachi doesn't win, because he is the strongest most of the time, he wins because he is intelligent and has observation skills that surpass a lot of people in the narutoverse. Sasuke may have the power, but he lacks one thing that Itachi has, patients.

Sasuke is patient to a degree, but its easy to get him riled up and lose some of the focus he has.

Does Sasuke have the potential to be greater then itachi in every way, i can't wait to see what Kishi has in store for him

the whole fight Itachi wasn't even trying to kill Sasuke, Sasuke fought with all his heart while Itachi was merely trying to get Orochimaru to show himself, Itachi even while on the decline was always several steps ahead of Sasuke, Totsuka blade and yata mirror all but guarantee his victory, keep in mind Itachi never wanted to kill Sasuke.

I agree that Itachi was not trying to kill sasuke. But my point is that even though Itachi thought he knew all of sasuke moves and had plans for them, sasuke managed to surprise and almost kill him with Kirin
