Gaara hasn't shown anything that would display him being on another level to his War Arc self so as of now Itachi wins with mid-high difficulty at worst. Sand attacks are blocked or overpowered with Susanoo, Gaara's defense is comfortably breached by Susanoo once Itachi gets close.
