There is also one tough thing called hardware ID ban. The only counter to that is having a spare notebook/computer. If combined with IP/subnetwork ban, it will be very hard to trick.

Though I'vet yet to see a hardware ID ban on forums. Only in online games.

For those of us with accounts we'd like back. I was thinking it could be like a once a year thing, that way members with permanent bans still have to wait at least a year.

There are bans that do last a year and aren't Pbans anyway.
