Madara's world was one of the worst: The waring clans era. It was truly a world without hope. It was truly Hell.
It was nothing like Konoha. One couldn't enjoy eating a sandwich or warm bath without worrying about being brutally murdered or someone they loved being sliced into pieces. Heck, it makes the Game of Thrones World and the Walking Dead World look like a cakewalk. Because at least they had momentary peace, however little it may be.
In Madara's world, one lives without any security, hope, or enjoyment. One is nothing but an insignificant slave to the crushing heels of violence.
Madara slowly lost his family one by one. Izuna was all he had left and was what kept him going in the world.
But his one sliver of hope was crushed when Izuna died. This was why Madara was driven insane. This was why Madara lost hope in a hopeless world.
So, yeah, Madara had it pretty bad.
Btw, here is what happened on a daily basis.
Little kids being beaten and tortured to death.
Madara most likely didn't only experience his family dying, had to live the the knowledge that their deaths were not quick and painless. He had to live with the knowledge that his family suffered immensely during their last moments, just like Hashirama's brother, Itama did.
