she had battle instinct... she is years upon years old, and people seem to forget that she was OP before she even ate the fruit, in the arc where she was dating that king guy, the whole reason she started being revered as a demon was because she redmisted a group of warriors by just looking at them... her battle awareness is there

While the gender is confusing, since 2011 that Neferpitou's chest is not flat anymore (At least in the anime).

Regardless, the only concrete evidence we have to determine Neferpitou's gender appears in the official databook "Hunter × Hunter Character and Word Data Book." In it, Neferpitou is referred to using the pronoun "彼” (kare) which is usually translated to “he."

As you said the 2011 anime may depict Neferpitou as a female by giving him a prominent bust so, it is possible to that Neferpitou's gender in the manga differs from the anime.
