Naruto never went full speed and itachi needed definitive the sharingan to keep up with him (phonas said base itachi stomps 3T kakashi)
Even Sai reacted to hebi sasuke and 3T Kakashi is definitive above hebi sasuke in terms of speed.

Itachi is fast, but not as fast as Kakashi wouldn't be able to react, he has feats which puts him on pair with itachi like keeping up with 6 gate guy, v2 jinchurikis ( their line of sight were even linked) and he kept up with obitos speed.
To be honest Kakashi even was able to blitz a surprised kaguya, but thats debatable.

Genjutsu wouldn't work against ration clone, who will paralyze itachi when both of them would grab each other like in canon,
after that kakashi finishes him with raikiri kunai.

Itachi wins considering overuse of the sharingan would cripple kakashi. There's no way a genius full blooded uchiha like itachi loses to a non uchiha with just one sharingan.
