Sasuke takes it. Itachi's best options(genjutsu) are non factor and his ninjutsu gets overpowered like in cannon. I don't see taijutsu going to him either.

Is Itachi sick? Basically will Itachi just die in combat? Sasuke was pretty much done by the end of the battle, and still had zero ways of defeating Itachi's susanoo.

-His Ninjutsu carry more power. (see Itachi's Katon vs. Sasuke's curse mark enhanced Katon) His exploding clones are tanked with CM2's wings. Tanked C2 albeit at the cost of a wing, but Itachi's Ninjutsu don't even come close to that level of firepower w/o Susanoo.

-There is no apparent gap in speed that'd let Itachi blitz or spank Sasuke in close combat, especially with the added physical boosts CM2 gives Sasuke.

-Genjutsu is a non factor as shown in canon.

In their canon fight the only thing standing between him and his victory was Itachi's Susanoo. Tsukuyomi and Amaterasu were dealt with. Without MS jutsus in play i'm sure Sasuke would overpower Itachi.
