well I am willing to bet it was because of his spiral brow since it`s the most uniqe feature he has .
may be his father or all of the members of his family have the same spiral brow xD.

If they could make out just from this feature then sanji should be a joyboy of void century and in fact there are more facts to support this like why his all attacks has DEVIL in the name like diamble jamble(Devils leg), Caesar in the latest chapter also told him "you devil" and all the facts which has been posted in all previous theories stating that he is son of CD/Gorosei could be conclude in the direction of him being the important successor of D Family, that pretty much explains his wanted poster too well..........If people can accept the shit like he is the lost son of gorosei then him being a joyboy shouldn't much be a problem i guess.........

One thing is certain, Sanji is not only going to be in the epicentre of this year's adventures he will get a massive powerup as well. Seeing how he is one of my fav chars (I would say 2nd fav one after Luffy), I'm so hyped about OP this year, haven't been this hyped since the timeskip.
