im gonna guess he died of natural causes, or probably due to great injuries from his battle with madara.

Nobody knows. Maybe his injuries against madara was severe and that couldve halved his life. Or he run out of chakra in a war and got killed by fodders. No different to the 3rd raikage

This^^, I think this was got off Narutopedia but it is not 100% official that this is how he died but it's probably the closest thing to go by

No, it was confirmed in Danzou's flashback that NiDaime along gave Hiruzen the title of Hokage. Kishi originally had both the first and second promote him, way back when Hiruzen died, than changed it later on. So the first was definitely dead when Hiruzen became the third.
