You can only phase through defenses. I don't thing you would be allowed to phase through a body like that at all. You can equally perform a Tai and cause the space around the body to move away from you. Reasonablly, you whole body goes where your arms or feet go.

Irreversible as in the souls sealed can't be unsealed? I think not. There is this uzumaki relic that orochimaru used to unseal the hokages. Maybe I did not understand you though.
