「～が気になっている」　doesnt mean "to be annoyed with～".
the basic idea of the term is "~ is on the subject's mind.=the subject is minding/caring/bothering about ~"
for instance if a boy says he has 気になっている女子(lit. a girl hes caring about),10 2 1 hes talking about a girl he fancies. Obviously thats not the case with sasuke.

Maybe sasukes uneasiness can be interpreted as annoyance but orochimaru doesnt say that....

Bottom line is

Sasuke is worried Naruto will get hurt by Madara

or

Maybe worried that Madara would kill him before he does

I think it's a different interpretation of what Orochimaru thought about Sasuke in 616. Like someone thought that the way the people who translated it was misleading or something.
