it doses't have to hit directly it has a vacum effect. being senju doesn't mean her skin is tough. it means she has a high amount of vitality. and like oro said if she looses blood and organs faster than she makes it she will die.

I don't know why Naruto would underestimate a kage level ninja yet fight seriously against a genin. What you are saying is true and I can't deny it but that would make Naruto a dummy.

Even still, we can all agree that Tsunade didn't go all out either and even if Naruto went all out, Tsunade would still casually beat Naruto up without taking any serious/extreme measures.

If her skin was as tough as you say she wouldn't have to activate her byakugou when using the third raikages teleportation thing. Being senju doesn't give you a body made of steel get that through your head if it did she wouldn't need byakugou. It gives you a big chakra pool and a long life. its the only reason she is alive(not even sure anymore) currently. his body was forcefully ripped apart and the same will happen to tsunade. 1v1 no kiba is miles below tsunade but this technique will 1 shot her.
