Kakashi killed his best friend which back then was Rin and because Kakashi's sharingan is Obito's this mean they are connected somehow! That's the way Obito got his MS because of Kakashi. ( you cant evolve only one eye )

But to get the MS you need to kill your BF not to see it. Kakashi didn't got it for him because he didn't know Obito was still alive.

Naruto is special because the Fourth sealed half of the Nine tails inside him. This way Naruto can't do the things the other bijuus can.
Sasuke isn't special! Itachi is more special than him but Sasuke got his MS by killing his brother not his BF!!
I don't think it really has much significance. To be real, we haven't see a very large sampling (see below) of Mangekyo Sharingans....how do we know it is exclusive?

Although Izuna also has red as the "background" I'd say more of the space is taken up by black. I dunno, I don't really think it is about what is the background and what is the design (I guess you're saying Sasuke has a black background and a red design) I think it is just the shape of the black design. If that makes any sense.

As posted above - and the poster beat me to it it, all designs are unique. Sasuke probably has a black one due to his proficiency with Amatersu.
