Zero knowledge works more in Shikamaru's favour since Sakura won't have knowledge on Shikamaru's various abilities involving shadow possession. Shikamaru doesn't need to know about Sakura's brute strength since he won't let her punch him whether he knows about it or not.

Getting close to Shikamaru without getting tangled in shadow possession isn't easy to say the least. Shikamaru's shadow shuriken possession technique where he throws a projectile at the victims's shadow to bind them will be very effective against Sakura if Sakura has no knowledge.

Both Shikamaru and Sakura are good at setting up traps and using ninja tools but Shikamaru trumps her in this category.

I say Shikamaru wins mid-high difficulty.

I would've given it to Sakura, but she seems far to slow to constantly dodge shadow technique. If she keeps it up, and has prior knowledge she can win this mid-high difference. If not, then I give this to shikamaru.
