Let's say that Sasuke kills his brother and is not encountered by Tobi. The Konoha ninja find him and they bring him back to the village. When he wakes up he decides to stay there and become a shinobi of the leaf once again. Think about how different the story would play out...
1. Sasuke "returns" honor to his clan (even though the higher ups know that Itachi was forced into the mission)
2. Sasuke doesn't fight Killer Bee (Kisame would have probably been sent instead seeing as he was the only other person for the job - if he doesn't succeed the summit doesn't happen so let's say he does)
3. Sasuke and Kakashi train together again, working with their Mangekyou's
4. Sasuke helps in the Pain invasion (no way to tell what would happen but he'd probably be pretty darn helpful)
5. The Summit is still attacked by Tobi and the war still starts
This would just be an interesting turn of events seeing as Sasuke would be fighting for the Alliance and thus he probably would not encounter Itachi unless he was with Naruto (and Bee is he escaped Kisame how he did Sasuke). This leads to a slippery slope of : Does Itachi still beat Kabuto, is the scroll found in Orochimaru's lair ever discovered, does Naruto even obtain anymore "upgrades" (if Killer Bee couldn't escape Kisame), etc.
One decision can really change this in the long run as you can see.
