Itachi was sick when he fought sasuke, and he wasn't planning on killing his little brother anyways but was more of giving sasuke a chance to avenge his clan. So he gave sasuke a good fight which he wanted to lose,but if it was a full health itachi with the will to kill sasuke would die lol
