Genjutsu on Kakashi so he would be controlled.
Rin betrayed the Leaf.
Kakashi couldn't see in all the rain, but not likely since he has a Sharingan.
Obito is just seeing an illusion/Genjutsu created by Madara.
Transformation Jutsu.
Rin tried to fight Kakashi for reasons that'll be stated later on.

My guess is Kakashi did it for the greater good; he has stated earlier that he couldn't protect Rin, and he doesn't do something which goes against his morals (such as killing Rin) without a reason. Then again, Kakashi does have a horrible track record for hitting his intended target (striking Haku instead of Zabuza in the first arc, or trying to his Deva Path but having the robot path in the way, etc.). Maybe he just missed again (and this subconciously triggers a mental disorder where he cannot hit his intended target).

When I first saw the thread, I thought you were going to ask for everyone's biggest upset so far, and that yours would be Kakashi killing Rin. On that train of thought, my biggest upset is Tobi is Obito.
