How did Madara really die?

So he wasn't dead after the Hashi fight.. We saw him with obito... but his dead carcass is shown in the sixth coffin that kabuto showed.. how did he die? And how would kabuto know he was really dead? And if tobi knew.. he shold have seen it coming that sixth coffin was going to be madara.

My guess is even tho the hashi cells kept him alive that long it cant do it forever.....so he just died out prolly 3 years after he met obito because i think thats how long it was b4 obito attacked the village,,,,,and madara dieing was what made him finally start the plan

he is hooked up to the giant hashi flower if you go back and look you can see it the hashi face and arms in the flower thing thing
