I think it would be tsunade and onoki will die. If tsunade dies Naruto will become next in line for hokage since he will be a hero of the ninja world after the war is over, or it will be Kakashi again.

No one. Madara gave up interest in fighting the 5 Kage. The 5 Kage, excluding Tsunade, does not have enough strength/Chakra in defeating Madara individually or as a team.

Exactly. She isn't really that relavant like the other four, also i think shes the weakest of them. I wouldn't be that surprised if kishi left them all alive though.
