Well it does, you'll notice a premium account easily cause their usernames are tinted yellow with a small star next to it

If you don't change your avatar it stays at the premium size, whether or not you have premium any more. Once you change your avatar, you won't be able to have a premium avatar till you win a contest or buy premium again.
