I got a little bit of thinking in my head here and got up with this idea. Its stupid and its probably wrong, but it is still just a idea.
I think I have found a way to explain how Madara could have use Perfect Susanoo against the first Hokage at the last fight and still no one that have seen the Perfect Susanoo have lived to tell about it. Also I think I might have found a way for Madara to awaken the Rinnegan.

Okay, now comes the real fun. The main reason I think that the First Hokage died in that fight at the Final Valley is because when you are revived by the Edo Tensei you will look the same as when you died, no matter how long it is since you died. When we see Oro revive the first and second Hokage against Hiruzen we see that both of them are pretty young. The first is as young as he was in the fight at the final Valley. So he either died there or somewhere close.
Madara was said to die there and its most likely taht he awakened the rinnegan shortly before he died. Now we know that to awaken the rinnegan you need both Senju and Uchiha DNA. Now Madara got that from the fight. So he might have injected it there at the spot, because he knew he was going to die and had already done the deal with Tobi (Probably) that Tobi would revive him years later. The Trauma I think Madara experienced was the Death of the first Hokage, the only man that Madara respected and looked up too. He might even have seen him as Madara's one and best friend, much like Sasuke and Naruto. When the 1st died, Madara awaken the Rinnegan and dies shortly after. Tobi then hides Madara's remains and leave takes the 1st's body or leave it.
This explains how Madara could have used the Perfect Susanoo against the 1st and still be sure that no one that have seen it had lived, and how he awakened the Rinnegan and was 100% that the 1st died.
