I've always thought this. Itachi from the get go had to become very strong and powerful. One reason was because he was a child during the time of war so it was his duty to grow strong and protect the village, his clan and baby Sasuke. There is a reason why Itachi at such a young age was able to beat a Sannin.

Later on he had the burden of choosing his village or his clan. He'd have to give information to the Leaf Village which would be betraying his own parents.

Hiding his emotions may have also taken a toll on his health.

Thoes stress lines also give him a look of wisdom, even at age 12, 16, 18 he looked like someone you could go to for advice about life lol. I think that was purposely done to go with his character. Despite being young he appears seasoned, sagelike.
