Why is Tsunade maintaining her youth?

The 5th Hokage is very vain and utilizes a transformation technique to maintain her youthful appearance. While it is minimal, that does waste some of her chakra. When putting her life on the line and fighting for the fate of the world, you'd think she'd be giving it her all and not worrying about appearances. Is it really going to kill her to be a little wrinkly?
