that line totally confused me?

He's going towards Sasuke. I suspect he's going to either poke his head or hug him or something like that. It will be something emotional.

He means he can still make a final jutsu to try and stop Sasuke from trying to destroy the leaf. The Koto Amatsukami he is under demands that he protect the leaf. Sasuke will lower his guard expecting the head poke and whammo.

Kabuto took measures to make sure he couldn't get caught in a genjutsu such as tsukiyomi (kabuto closed of his eyes, so genjutsu through eye contacts wasnt going to work), even then, kabuto would've broken out of that, even though this is itachi we're talking about.
