no he can't recover from the damage, why would you think tsunade prohibited him not to use that jutsu ?

because if he don't throw the rasen shuriken then the attack will be a melee type, when it hit the target it also hurt the user chakra system thus will make him retire from being a ninja,

and you're biased by restrict sasuke sharingan in base mode.
if you want to see how base sasuke and naruto fight will end up then you should see the manga where naruto and sasuke meet for the 1st time in part 2,

idk about that but without sharingan he become number 1 in the academy, sharingan is just a tool
having a sharingan it doesn't mean you will get auto power up, you need to train yourself to use that eye properly
