Well I would assume you would need a 3 tomoe to begin with. x.x Anyway it doesn't say. Just that you need to be a member for 4 months and take a test given by nexus.

I believe you need 3 tomoe or you wont even be considered, thats when Crow tried to get madara and he only had 3T

To get Madara you need to pass a battle test given to you by Zenryoky, Izuna or Goro. However that bio is suspended at the moment as new abilities are being shown in the manga.
To have MS you can have it by two means: either being a sensei and having permission from the RP Mods and Nexus (i believe you might also need Rei's) and taking the test; or you can pass the MS tournament and get it as a prize.
To have EMS (Madara) you need to fulfill the requirements for MS meaning that i think you need to be a sensei and have permission to take the test.
However, all is halted sooooo...no good! =)
