My question is:

In theory couldn't Sasuke simply Ameno someone's heart out of their chest?

Imagine if he just use them in his fights that causes to confuse his opponents to swap places with him, cause that would be pretty dumb badass and tactical, and can add variants to his fighting style like that.

He hasn’t used more than what it was before as an adult did he? I remember him using it 3 times before cool down..

The ‘3 time usage before cool down’ is completely fan made. There’s no mention of that in the manga or databook, so we can’t just consider it as facts. We can speculate and say it could be a cast of low on chakra, or new to his Rinnegan powers.

What we do know is as a adult, Sasuke is able to use Ameno better and more efficiently. Sasuke use Ameno with his chakra drain from traveling to the shin dimension. His chakra was drain to a level that he couldn’t activate his EMS or Tomoe in his Rinnegan.
We also see him using Ameno without looking at objects he’s swapping to. And swapping places with jutsu.
