Sasuke attained Perfect V4 Susanoo in Danzo's fight. Why didnt he attain Perfect, then and there?

Because he didn't attain it nothing happens instantly unless you Clone (Boruto+ add pulls). That's like me saying why did Sasuke start with ribcage Susa and not V4 Susa.

Madara only achieved Perfect Susano'O after grabbing Izuna's eyes. Definitely a connection there. Itachi didnt achieve Perfect Susano'O. See my point?

Rokudo's power was never stated as being the reason for perfect Susa and the databook entry also confirms M&S is only necessary. Do toy have anything to confirm other than that?

Ms users most likely don't have the chakra nor ocular power to hold perfect susanoo, Thus why most MS users we've seen stop at a complete susanoo. It's not impossible for a MS user do have it but you most likely need to be born like madara, with large chakra and extra powerful chakra even by uchiha standards and for kakashi, 6 paths chakra bridged that gap allowing him to use it, like EMS bridged that gap for sasuke.

Oh and btw it was kakashi who stated 6paths chakra increases ocluar powers.

He would spam MS without any major drawbacks or chakra problems, Unlike his MS form
He could hold susanoo for much much longer periods without many major drawbacks
His control of enton seemed alot better
Overall stamina seems a lot better
I'd assume his genjutsu got a massive boost along with all his other ocular powers
Sharingan was precog got a massive upgrade from not being able to track v2 raikage to tracking Juubi jin obito
