There is a bunch of differences between those two and their relationship with Boruto but primarly you can wrap it around the fact that Mitsuki's relations is to gain emotions and character, while Sarada relations is to express her emotions to Boruto as a character.

Mitsuki sees Boruto as a figure that will lead him to finding himself.

Sarada likely saw similarities in Boruto to herself but eventually also gained other emotions that we never see Mitsuki express (since he is still a test tube kid that is learning about what is right and what is wrong and what traits to have and what to value.)

Unlike Mitsuki, Sarada goes through another relational development and usually uses that relation to try and get close to Boruto but also try to help him due to her feelings for the said character, as well as sharing some bond.

Not really like Hinata to Naruto but some essence is there that triggered it for Sarada.

You know for a fact that they aren't going to make a last name as long winded as Uchiha-Uzumaki.

Usually in Naruto the child carries the father's last name and the wife adopts the husbands last name.

The only exception to the rule are three scenarios

• Naruto being the son of the 4th and thus for his safety being called Uzumaki (as well as his mother's last name
  carrying more weight in clan dominance). Or a child not knowing who the father is and therefore adopting his
  Mother's last name.
  
  
• Mito and Kushina keeping their last names respectively.
  Due to the budding alliance between Konoha and the Uzumaki (in order to keep good terms), as well as to having a
  more noble clan name then Namikaze.
  
  (And even then for Mito at least her children and her grandchildren adopted the Senju name.)
  
• Sai not having a last name and therefore because of that adopting Ino's clan name.

I mean you can try and argue that she can keep her Uchiha last name due to it carrying some weight but the whole Uzumaki-Uchiha as being a surname sound ludicrous.

Even then I doubt Sarada will keep her last name since the Uchiha barely has any influence now compared to Naruto being a Kage with his father being one as well.

I mean I assumed Hinata would keep her last name since the Hyuga are currently the most influential and wealthiest clan in Konoha but she did not and her children didn't either... even though they still share bonds and relations with the clan itself.

I expect the same for Sarada even if she is one of the last Uchihas.

it isn't like Sasuke reps his clan anyway and wishes for it to be known.
